Who are we?

We are an Incorporated Society, Charitable Trust with a vision to provide better health and wellbeing for everyone in Porirua with a focus on reducing health disparities.

 

 

What do we do?

We develop and implement initiatives to improve health and disabilities outcomes to reduce health inequalities for Maori, Pacific peoples, and people on low   income. 

We provide services to facilitate achievement of the recommendations from the Porirua District Health & Disability Report and Plan 2000 

 

 

How do we do it?

Our key focus areas come from the PHLT Strategic Plan which have been incorporated into an action plan that is guided by four streams:

  - Weekly All Points e-Bulletin

  - Porirua Healthlinks Trust Website

  - Media (Kapi Mana, City Life Newspapers)

  - Creekfest

  - Radio (Hollah FM, Atiawa Toa FM)

 

  • Healthlinks

To develop the capacity required to contribute effectively to the organisations strategic  objectives through:

  - Strong Governance – Community voices

  - Effective Staffing Capacity

 

  • Health Outcomes

The life of the health sector in Porirua is extensive, understanding trends and forecasts are an important part of appreciating the past, present and future of health issues for Porirua City

  - Porirua Alcohol & Drug Cluster (PADC)

  - Porirua Transport Cluster

  - Diabetics Cluster

  - Porirua Healthy Safer City Trust

 

  • Dialogue – Advocate for improved health services by

Maintaining and developing effective relationships with Maori, Pacific peoples, minority, new settlement groups and mainstream through meetings with:

PHO’s, Pacific Health Service, Refugee Settlement, Advisory Meetings, Advisory   Groups, Older Persons Advisory Group, Ratepayers Associations, Porirua   violence prevention networks, Porirua Healthy Safer Trust, Kenepuru   Community Hospital Advisory Committee, Maori Post Suicide Advocates and   facilitating events such as Creekfest.
